The Annual Joint Health and Social Care Self Assessment was carried out on how the Council meets the needs of People with a Learning Disability and their Carers. The assessment focussed on the period 1 April 2013 to 31 March 2014. The final submission was agreed by the Learning Disability Partnership Board.
